Community Development Initiatives collaborated with numerous agencies and community-based organizations in 2015 to conduct a Community Health Needs Assessment. This project sought to identify and prioritize health needs of vulnerable, low-income populations in a 20-county region of West Texas.
They collected and analyzed health data, invited leaders from the communities to help prioritize identified needs, and synthesized the findings and key informant feedback into reports for each of the 20 counties, as well as a comprehensive report for the 20-county region.
They have created web pages to host each of the reports mentioned above, as well as asset and thematic maps of health resources. They envision communities will use these documents and maps to inform strategic planning and related efforts.
